Enrollment

The first step to joining the Nittany Lion Battalion is completing the required enrollment paperwork. Please complete the forms below, using the provided guides.

Medical

DA 3425 – MEDICAL FITNESS STATEMENT, APR23

All Cadets must be cleared by a medical professional to participate in ANY physical training with the Nittany Lion Battalion. Cadets can easily achieve this by having their Primary Care Physician complete the DA Form 3425.

*** The only exceptions are Cadets who’ve been QUALIFIED through the Department of Defense Medical Evaluation Review Board (DODMERB) while previously competing for a Service Academy – OR – the National Army ROTC scholarship process. If an individual is currently in the process, but not yet qualified, they need a completed DA Form 3425 to participate in training. ***

Contracting Documents and Resources:

Cadets must still replace all the ‘Electives’ with actual Penn State classes, in order for the 104-R to be valid.  Once the form is complete and accurately reflects an individual’s academic progression and graduation date, the Cadet and their Academic Adviser must sign on Page 2.  The Professor of Military Science will sign, after all other portions are completed.
