• USACC 139-R Cadet Enrollment, OCT23 TUTORIAL FOR 139-R

~ This form is utilized for all Cadets enrolling in our program and has specific instructions to complete. Please follow along with the instructions as you complete the 139-R.

• CC FORM 137-R (Access to Student Records) ***

~The 137-R must be completed by all Cadets enrolling in Army ROTC and requires a signature in the “Authorization” section of the form.

• DA FORM 3425-R (Medical Fitness Statement) ***

~ This form allows Cadets entering the program to participate in our Physical Training sessions. Exceptions for those who are already DODMERB or MEPS qualified may apply.

• USACC Form 136-R Government Sponsored Benefits***

~The 136-R must be completed by all students entering the Army ROTC program.
